At SportsMedicine Partners, P.C., we are dedicated to providing excellent medical care to each of our patients. In addition, our billing department is dedicated to providing prompt and accurate billing to your insurance carriers. Please read our Financial Policy Statement.

We ask your help by providing us with the most accurate and up to date information so that we may minimize the number of billing errors and confusion.

In the ever changing insurance industry it is imperative that we have the following information to allow for accurate and timely billing.

• All current demographic information. (Please be sure to completely and accurately fill out the applicable forms.)
• If the patient is a minor, we will need all of the above information for both parents and the responsible party (i.e. parent, legal guardian, power of attorney, etc.)
• Current insurance information. We will need to scan your insurance card(s) in addition to a photo ID. Please bring these to your visit.
• We now offer our patients the convenience of paying their account balances online through our website. If you would like to pay your bill online, please click here.
